First, you'll need to go to your doctor and tell him you want a reduction. He will refer you to a surgeon, and he will take photos of your breasts, measure and send a report based on your medical records and complaints to the insurance company.

The women that I know who had extra large breasts had a lot of back pain. Visit an orthopedic doctor and he will recommend a reduction.

i am not 100% sure of this, but my friend had it done some years ago.
i think it is considered a COSMETIC SURGERY that is also considered VOLUNTARY unless it is related to breast cancer, if that's the case, you would need written documentation of the fact.
