Decide in which state you want to incorporate, then google the state name and "division of corporations" This should take you to the secretary of state (or equivalent) website, which is where you would file articles of incorporation. There is usually a link to forms. Download and complete the document entitled "Articles of Incorporation" and submit to the SOS with the appropriate filing fee, which is usually referenced on the application. Once this is filed, you are incorporated.

Depends on where you are located. This is something you really can't and shouldn't do yourself. Hire an attorney (who will first guide you through the process and help you decide whether you need to, or want to, be incorporated). It costs money to maintain a corporation, and there are significant tax implications.
