This sounds like something a friend of mine went through with her son and it turned out there was something abnormal with his intestines. With treatment he is fine and is now a happy 12 year old. I would go to another Dr. and if they are stumped then go to another one. It's not normal and it could be something very simple.

One of mine had this problem too. Inspite of healthy diet and plenty of fluids, she had a really hard time. I had a niece that had the same problem as well. When it hurts them to go they will hold it longer, only making the problem worse. Dr's didn't really have any other answer than "they aren't drinking enough water or juice". Well this wasn't the case for either. Any of their "textbook" answers just did not fit the situation. It was very frustrating. Finally my sister was able to find a doctor that could help. Some children's GI system doesn't have enough of the "healthy bacteria" established yet. This can cause difficulty eliminating for them. Plenty of fluids is important however, your childs system may need a little help in balancing it's "flora". Here is what actually worked.
Go to a health food store and find some "FOS" in powdered form. This stands for "Fructo-oligosaccharides", it is a natural sugar found in vegetables. It is only partially digestable in humans (so you don't need to worry about it adding to your childs sugar intake, this is different). This feeds the good bacteria (such as Bifidobacteria and Lactobacillus) while simultaneously inhibiting the harmful bacteria. It will safely improve your childs colon function without the use of laxatives or giving him inordinate amounts of fruit juice (high concentrated sugar).
You can start by mixing about 1/2 teas. in his morning milk or juice daily. He will not detect it. After about 5 days increase amount to 1 full teaspoon daily in his morning drink. This just helps ease his little system into it. It is natural and gentle and it may take several days for you to notice a difference. However, you will be amazed at the genuine improvement. You just continue to give him this until he is a little older and his little system seems to be balanced on its own.
One more note. Different brands seemed to work better than others. I can't remember the brand that was the best. It was more expensive, harder to find and it came in a small clear plactic jar (sorry so vague). I do remeber using the "NOW" brand and it was just okay, the other worked way better. I got it from my sister who got it from the doctor at the time. He was a medical doctor who had gone into holistic medicine and he sold high quality herbals and things. So you may need to go through a place like that.
Okay, good luck. Please try this, it will help your little one.

Aside from cheese, too much milk or too much apple juice or too much banana can cause hard stools.

Keep in mind that the following foods are recommended for diarrhea ... the BRAT diet
Bananas
Rice (plain white rice)
Applesauce or apple juice (cooked apple products, raw isn't same)
Toast (plain white bread toast, no butter).

These foods and similar (plain white pasta for example) all bind the stool and make it harder, hence why it's recommended for loose stools. Now, you have the opposite issue, therefore you might do well to avoid those for a while.

I agree with trying prunes or juice and extra water (yeah, easier said than done). Also maybe a little fax seed oil in some food.

Is your baby on formula or regular milk? My son had the same problem until he was almost 2. They found out that cows milk was too hard on his stomach and he could not digest the enzymes in the milk. We switched to goats milk and limited the amount of milk he drank and he got better. You might just try limiting the milk you give him and give him more juice to loosen him up. I took my son to the doctor and they didn't do anything for him. Good luck, hope this helps. It worked for my son.

I would take him to the doctor and have him checked out. Is he eating enough fruit? Kids need a well-balanced diet, that includes fiber. Also make sure that he is getting water daily too.

Are you sure he's not being sexually assaulted. I know it's disgusting, but I know of one baby that had trouble passing stool, and they found that she was being sexually molested by her father. It took almost a year of healing afterwards for her problems to clear up.

BTW, she is grown now and has no memory of this, so none of us remind her. (Her father died at his own hand before he could go to trial.)

Is he formula fed? Try giving him things like oatmeal, grapes, prunes ect. I nursed mine and it helped a lot.
