The CTR is the cardio-thoracic ratio

I'm assuming we are talking about a chest X-ray

The CTR should not be bigger than 50% because this indicates dilation of the heart (medical term: cardiomegaly). Cardiomegaly is bad because the heart muscle is not pumping as well as it can (just like the muscles of your arm work best when they are in mid position and are weakest in full stretch, your heart muscle is more ineffecient if your heart is dilated. However a CTR 45% is okay.
