The State of California keeps that information on line so schools and doctors can check if parents lost record. I don't know what state you live in and not sure if your state have this service or not. If not, ask your school or doctor. The school will for sure have records because they cannot let your daughter enroll unless she has had all her shots.

Contact her doctor. They have an immunization record, and they wil l be happy to give you a copy.

Her pediatrician would be a good source. If she's seen more than one, or been treated at a clinic, call them as well.
