It's very self-xplanatory. Disease control involves curbing the spread of the disease and is a short-term process. Eradication is a long process and it involves vaccination to make sure that people are protected from the particular disease.

Disease control is just that. The control of a disease so that it does not get out of control. However; erradication is a different but not a completely different concept. Whereas control involves just that, control, erradication involves the complete annhialation of the such disease and is usually practiced when a disease becomes too dangerous to simply control.
