It some what depends on the software you used to print it. The overall name is simply "report". If it shows a graph then it a called either a "graph" or "visual display report". If it shows both it can be called a "combined presentation report".

If it shows all the records it is called a "detailed report". If it shows groups of records in a summary format, it is called a "summary report".

There is no particularly wrong or right way, but the best idea is to keep it simple.
