I spend a lot of my income on my and my family's health to stay alive longer. The same money could obviously help others a lot more in a poor country (vaccines, education, etc.).

Which should I do?

Perhaps there's a middle ground where you can meet your obligations to your family's health and still donate to help others who are less fortunate. Then taking care of yourself is prolonging the life of a source of healing for others.

the question to me seems like a poor excuse . helping the poor overseas cost such a nominally small amount why not just commit a few dollars to a noble cause on a monthly basis and enjoy a long life also ...

Spend it on living longer. A dead person can't be of much help to others, unless your cafeteria is serving Soylent Green today. You don't need money to help others, you could volunteer your time and efforts instead.

if you are meant to die and you are squandering money to prolong the enevitable, you are selfish but apparently not to far gone if you have the thought to help others. please, if i had money and knew i was dieing and could only prolong my life, not save it, the money would go to help save someone who would get well. you cant take it with you to the other side, no one cares what you had on earth.
