Failure to reduce, That is, breaking a complex series of events into smaller chunks of reality. As Sir Aurthur Conan Doyle wrote in the the voice of his Immortal Detective Sherlock Holmes " Eliminate the Impossible and Whatever is left, However improbable, is the Truth."
and finally Acceptance, Realizing that the final solution is the actuality. and does not require further reduction.
The First blinds us to the true question, the second is made in haste, and the Third mires us in inaction which only makes the problem more intractable.

Attitude or inner approach to a problem is the most annoying barrier to identify and understand a problem.
If we have positive attitude, then definitely we can embrace the chance given to us.
If our attitude is negative then, we build the problem to such an extent that it becomes impossible to solve and we start acting as an victim.

Foucault exlpains it best- Our inability to remove ourselves from the disciplines/institutions we have been raised within or incorporated into- examples: education, medicine, ideas of "normalcy"- including anything from as large as a racial construct to as small as the idea of the family, or even what constitutes a "normal" self.
