I guess no need. For ethical reasons a doctor with HIV should refrain from dealing with his patients especially if he needs to perform procedures that involve body fluids. Right to confidentiality should be observed.

no, just beacause he a doctor, it doesn't keep him from the same human rights as everyone else. By law, it's nobodies business. Although, if a doctor performes a surgery or transfusion, the patient must be notified.

I do not know WHO WOULD require a doctor to report it to patients and still be in practice.
The American Medical Association would probably be the place to ask this question.
If the doc does NOT report his disease, then no one WILL require him to report it, it will be up to the doc's HONESTY alone to do the right thing, scary, huh?
