Well, nothing says "I don't love you anymore" like a protective order. But even that's not working for you.
You can either turn him in every single time he violates it, or move without a forwarding address. It sounds like he'll burn out completely in less than 3 years anyway, so it's not forever.

You have done all you can for him. You have stayed until you are in danger of losing your life. Now it is up to him to change his ways or die a lonely man.
You don't need to make him see you are done with him. Just get some friends to help you pack and move. He should not be anywhere near you if you have a restraining order on him. Do not give him any information about where you are going. Don't give him your phone number either. Get a lawyer and get a divorce quickly. You need to be free of him for once and for all.

You need to tell him in a calm manner(watch your mouth) and firm voice that it is over. The time for second chances has run out. You feel sorry for him, but a relationship based on pity doesn't work and you assume that's not what he wants out of a relationship either. He needs to let go and go on with his life and hopefully he will seek help, because in your heart and in your mind you already have let him go, therefore it is really finished.
If he cannot let go you'll need to call the police. Be careful and Good Luck.

You already have the protective order, that's a good start.
I would suggest one conversation, over the phone, saying that you've put up with enough and now that he's started hitting you then that is it. I would suggest filing for divorce as soon as possible. He needs to get his act together and you don't need him taking you down with him.

By not having any contact with him. By filing for divorce. Hang tough. Call the cops if you have to. Don't let him back into your life. You have the power to end this marriage. Life is too short to stay with somebody who you're only going backwards with. Good luck.

Have you moved out yet? I think the only way you'll get him to understand that you mean it is when you move out. Otherwise, just have nothing to do with him. If he calls, hang up on him. If he writes to you, send it back to him. If he shows up on your doorstep, call the cops.
And honey, you were a good wife. Nobody has a right to lay a hand on you. Good for you for leaving him. Stand proud. :)

do not talk to him or see him no matter what, if you have to move away for awhile, you never know what people on drugs are capable of.
