It is when you undertake something, be it a job, hobby or volunteer work, homework etc. and not only do it to the best of your ability, but go beyond to really have the situation having been handled not just well, but SUPER well.

Example of something good: your neighbor had surgery and you sent flowers to her at her home with a nice card.

Example of going the extra mile on something good: your neighbor had surgery and you sent flowers to her home with a nice card and also showed up with dinner that night and organized the neighbors to make meals on a rotating basis for her. You also did some routine household chores for her and let her know when you would be back to help/visit again. Looking out for her until she is healed and independent again.

Some people go the extra mile on a regular basis, that is just how they are. Others learn and continue from there. Some never do.
