There are two important sources to consult.

One is the human resources office at her job. She should ask about the Family Leave act and also ask that they check her benefits coverage for short-term disability insurance which she may have opted for. Worker's Compensation pays disability if the cause was from or--sometimes--merely at work. Short-term disability coverage is for all the rest of the time (within the limits of the policy, wounds or injuries from skydiving over southern Lebanon during the war might not be covered).

The other is the social security administration. Look in the blue pages of the telephone book, the government listings, to find the office near you. They will give you some help, directions, or instructions on finding a lawyer or other course of action.

Good luck, but between the two you should get help. Start with your job, but if you don't have insurance then go to the Social Security administration. Your state, and some cities, will also have assistance programs so listen carefully when someone gives a suggestion--it isn't the proverbial 'run around' but there are programs for specific ranges of situations and if your mother fits, fine, but if she doesn't then she doesn't.

if she took out short term disability on her job then all she needs to do is get the paper work from her job and take it to the doctor he by law HAS to fill out the paper work....unless of course he's saying there is nothing wrong with her and she can work....then there is a dispute and he doesnot have to fill out the paper work.
The original doctor that saw her should be the doctor that fills out the paper work. There has to be some other reason why he is not filling out the paper work.

Typicially, it's the company that should have the short term disability forms for a physician to fill out. Whether she gets it or not is up to the companies insurance provider

Short term disability is done through the job, not the doctor. Not all companies offer it, and sometimes you have to pay into it first.

If a Dr wont fill out this type of paperwork, which is quite important , then your mom needs to get another Dr. Its rediculous that he will not help her with this. A patients financial well being is very important top a persons recovery and general health... find Dr that understand this.

Just keep calling doctors to find out what ones will fill out the papers as long as she is not trying to collect workmans comp(they have their own doctors) the doctor should fill them out. It sound like to me that these doctors don't believe her or think her back isn't hurt enough to be off long enough to collect short term disibility. Hope this helps you out hun..........Flo

check with human resources in her company first, if your mother had fees taken out of her pay for STD they can give you the paperwork to fill out. ask them about her company health insurance also and have them explain what is covered. many people short change themselves on STD so if she has it call and have a list of questions you would like answered.
