It is assumed that during the days of vaudeville, you had dancers on stage all dressed up and one wore a banana costume (of real bananas)

One fell off and landed on the stage.

As the next comedian came out, he didn't see the banana, stepped on it and slipped, making a great pratfall.

the audience howled.

Other say Hal Roach Sr, saw this happen in the early days of movies when people would have a banana, but were careless when they threw the peels. Someone came along and WHAMMO.

This is an early use of that skit:
In 1906 a comedy record featured a popular character of the time, "Cal Stewart" saying:
"I don't think much of a man what throws a bananer peelin' on the sidewalk, and I don't think much of a bananer what throws a man on the sidewalk, neether. ... my foot hit that bananer peelin' and I went up in the air, and *** down ker-plunk, and fer about a minnit I seen all the stars what stronomy tells about, and some that haint been discovered yit. Wall jist as I wuz pickin' myself up a little boy *** runnin' cross the street and he sed "Oh mister, won't you please do that agin, my mother didn't see you do it".

Also, author Jules Verne is said to have coined the phrase, "In this crazy, mixed up world the banana reigns supreme". Hence, going "bananas" has become associated with meaning going "crazy" in today's culture
