I guess because of loose skin that develops over time. The lack of body firmness.
But it's all a big shame, very disrespectful.

I always thought it was "old hag". Maybe there called both, but either one is not right. No one should call and elderly lady or eldery man such names. We should respect older people. They have lived a long life and when things are harder when you are older and things take more time.
