I just went through this a year ago! First and foremost, you need to contact your local Social Security office. You will need to take your marriage license, form of ID, birth certificate, and possibly even a utility bill as proof of residence. Once you do this, they will take you through the step-by-step process. You will receive your new SSN card in the mail within a few weeks. After you receive it, you will want to go to the DMV and update your driver's license. You will need your updated SSN card in order to update your license. Once those two major things are done, you can start contacting your bank and other agencies for the name change. You may even have to get a new bank card issued to you which can be tricky if you use your bank card often. As long as follow in this order, you will find it much easier to make the change. Since it has been over 2 years you may have to go through a name change at your county's Superior Court. Once that is done, then you can take the order to the social security, job, dmv, and every place else you have to change it. But I would start at either the dmv or social security office and follow their guidelines.

You need to get your name changed at every individual place that has your name on file. So get your marriage license and MAKE COPIES>
Social Security Office
Passport Office
DMV for your driver's license
ALL banks
ALL credit cards
Any financial institutions where you have an account, 401(k), IRAs, Mutual Funds, anything
YOUR employer
ALL doctors offices/Dentists, eyedoctors (when you have your new health cards)
Health Insurance
Any professional Societies you affiliate with
it takes a lot of work, but anywhere you can think of that has your name on file, you need to notify them
Most of them will take a copy of your marriage license, but the Social Security office REQUIRES and original copy and they KEEP IT, so go to the county courthouse and get more than one official copy. Your passport will require an original copy, but I believe they send it back.
