Basiclly measure from the base of the penis to the tip of the head alone the top surface (the part that you see when you look down)

If your a girl measuring her partner, measure from the base to the tip on the top side.

If your a guy measuring your own, measure on the bottom side, anus to tip, adding 10% for self-satisfaction, then write down 9 inches.

Measure it from the abdomen side when it is in erect position.
