Unfortunately, there are thousands of different spyware and adware varieties out there that you would need to watch for and the steps for removing most manually can be anywhere from simple to very involved.

This means that manual maintenance is really not an effective choice. Although you want to avoid downloads, a couple of decent utilities for this are Spybot and AdAware.

Spybot: http://www.download.com/Spybot-Search-Destroy/3000-8022_4-10401314.html?tag=lst-0-1

AdAware: http://www.download.com/Ad-Aware-SE-Personal-Edition/3000-8022_4-10399602.html?tag=lst-0-1

Also, check that your Hosts file has not been corrupted. It can be found at either \\Winnt\System32\drivers\etc or \\Windows\System32\drivers\etc

You should most likely have nothing in the Host file below the "127.0.0.1 localhost" line. If there is a bunch of stuff in there, try to find another, uneditted version to copy over.

There is not straight answer to your question. As you said, spyware/worms/adware will try to disguise themselves. A list on the website will be not updated frequent enough to catch all of them. However, these spyware (and etc.) have signatures which can be detected through scanning the registry in the memory. Therefore, it's more effective to use one of the anti-spyware software to get rid of them.

However, if you see a process that is suspicious in your process task list, you can try look up this website:
http://www.liutilities.com/products/wintaskspro/processlibrary/
Those that have been identified as a threat to your system will be listed there.

For anti-spyware sofware, you can try Lavasoft Adaware (http://www.lavasoftusa.com/software/adaware/). The Personal edition is free for non-commercial user.
