No energy is ever lost, that would be in defiance of the laws of physics. Ineficiency in a machine is any energy expended doing something other than what the machine was designed to do.

Take for example - a car engine. It is designed to turn a flywheel as fast as possible. However, whatever the horsepower of the engine actually is, it will be less than the potential energy calculation of burning the petrol (on paper) says it will be.

That is where inefficiency comes in. Some energy becomes light inside when the petrol burns, a lot of energy becomes heat, a small amount of petrol won't get burnt at all and will come out as light/heat and smoke as it goes up in the exhaust. And finally, car engines aren't silent are they? A lot of energy becomes sound.

In short any of the enegy expended in these way is not turning the flywheel - and that is where you get inefficiency.

The inefficiencies in machines may heat losses due to friction, basic inefficiencies in the thermodynamic cycle used (the heat of combustion of a fuel can never be entirely converted into mechanical energy) and less than ideal performance of mechanical parts on a fluid (e.g. in a pump inevitably some energy is lost in heating up the fluid rather than producing pressure). The energy is wasted in that it is not part of the intended output of the machine but still exists in the form of waste heat. Vibration and noise are ultimately dissipated as heat.

No. It means that the energy you want to use as mechanical energy instead become heat energy, and does not do any work in the machine. Conservation of matter and energy - check it out.

Energy is being absorbed by things like friction (producing heat) or just resistance (kinetic energy being transferred to the air around it). It is lost in the idea that it is not being put towards the intended purpose of the machine.

all machines have a certain degree of inefficiency. in the context of physics, inefficiency refers to the fact that whatever amount of energy you "input" into a machine to do work, it always "outputs" an amount of energy less.

the energy in this sense is not actually lost. the difference between your input energy and your output energy goes into negating physical forces such as friction and gravity.

for example, when you ice skate, the force you exert to push yourself forward 10 meters will not take you 10 meters across the ice, but rather somewhere shorter, like 9 meters. this is because some of the energy you exerted in pushing yourself is spent counteracting the friction between your skates and the ice.

Energy can never be lost in classical physics. It can only be tranferred in to another energy form. In machines enegry is wasted manily in the form of heat or sound. That is why mechines get heated or produce sound when they are used.

no, it doesn't mean energy is lost.
waste energy means the energy that has not been utilised for the required purpose.
e.g. when a bike engine gets heated it is because some energy of fuel is turned into heat energy, which was not desired

efficient machine is not 100% efficient.It is 60 or hardly 70% efficient.There is no such a machine which does'nt waste energy.
