The most common problems are external, like ich and other parasites. When they start to suffer from this (and even before you can see the problem) they will scrape themselves on the substrate/plants/decorations.
Other indications of problems are weight loss, appetite loss and odd swimming/balance problems.
Most people get to know the habits of their fish so well that any odd behavior is quickly picked up on.

isit lying on its side or anything??
check for any change in apperance e.g spots or fin rot

In my experience it will start swimming backwards. I would quarantine the poor little thing so that that the other fish are protected.Then I would bring it to a place that sells fish because they may have a medication to put into the water that will save it's little life. Be sure to ask if there is a medication to treat the aquarium water as well. Best of luck.

we can get to know that the fish is ill if it is struggling to get out of water { as she might be feeling cold in the water }

she wont behave naturally....she'll not eat the food u'll give....or elz if u ve many fish she mite not b interactive towards them....I m too having fish at my home thts y i m saying this!

if its inactive and is being pestered by other Fishes of the neighbourhood.
