I think I rival a goldfish for the amount of time concentrating on one task and i'm becoming more and more aware that it really hampers my productivity. I've tried focussing on one thing at a time, but it doesnt take long to switch back to goldfish mode without even knowing it

You should practise various techniques that will increase your ability to focus. Active reading is just one example. You might also schedule an appointment with your physician. Attention Deficit Hyperactivity Disorder is a real medical condition and there are quite a few medications availble that may help you concentrate. ADHD is a recognised disability under the ADA and can be quite debilitating in some patients. Sounds personally familiar. Also sounds like you have the potential to have laser-like concentration if you will it to yourself.
What I do, is I practice, practice, practice - go inside your closet,
turn off the lights. Hold a pencil or a small item on your hand and do your best to maximize your knowledge about that one item. If it's a pencil, learn everything about that pencil. The psyche would be - if you and that pencil are the only ones left in this world, what would you do? This is my personal exercise. I've outgrown the closet. My brain shuts down from the chaos outside and I pinpoint my focus on the task ahead. It'll take you several days, weeks maybe months. But with persistence, you'll notice the improvement.
