Depending on if the restaurant staff is trained in the "proper" way of serving a table, placing your silverware at the four o'clock position will alert the server that you plate may be taken away. Places like Appleby's are not "fine, haute cuisine," you know? Not that they don't serve good eats, but the level of service is not meant for this type of training of the staff.

Dr.'s answer was a little harsh, and he is the reason people think food gets spit in it, because i'm sure someone has done it to him!!

anyway, as a server, put your fork/knive together on your plate (laying right next to eachother), and push it to the side of you just a little bit. and it's always nice to hand the plate to your server also, instead of just sitting there while they strugle to reach it. ;)

On the plate, not too picky. If he doesn't notice, reduce the tip.
