An intern and a fellow are both Md's. However, one does have more experience than the other. An intern is a medical doctor who has just graduated from medical school and is just beginning his practice of medicine. This lasts 1 year and they go thru several rotations (moving from one specialty to another). Most of the time it is during this year that many will decide what medical field they like and are most suited for.

A fellow is one who has been thru his internship, and his residency (where he betters the skills in his particular field of study) and is now in a position to teach doctors who are just beginning their practice of medicine.

MD is the doctor, intern is a medical school student, and a fellow? not sure on that one.

An M.D. is a physician. An intern is an M.D. in training. A fellow (in US) is a guy, any guy.
