Because it is dirty. Not the people, the country is however. Take Mexico City, living there is the same as smoking something like 20 cigarettes a day. I think it's just dusty, but that's largely due to its climate.

Saying that people are dirty is just a way of voicing prejudice. People are generally as clean as they can be given their access to resources.
