Yeah - I had a c-section and the epidural wore off. Not good. Not good at all. Apparently I'm a bit 'immune' to some anaesthetics as well. They ended up paralysing me from the neck down in order to do the section - that's how much anaesthetic it took. I was terrified.

Speaking as a dentist:
The most common reason for LA not working is inaccurate placement. Nearly always with a regional block where positioning is critical.
Secondly, patients sometimes live for weeks with a toothache until it is unbearable. This is what we call a "hot" tooth, and it is thought that the increased inflammation interferes with the action of the LA. This requires additional techniques to fully numb it.
Thirdly, there can be anatomic variability of the nerves e.g. double branches.
Fourthly, emotion is a factor in any pain experience, and some patients will be more agitated than others. If in doubt, I do a dummy test of the LA effect without telling the patient what to expect. The use of nitrous oxide or pre-med diazepam may help them.
Be careful of "huge" doses, especially in plain LA solutions (usually used in medicine). There is an overdose limit, and people do die - usually children where it is easy to OD.

WHAT!!! ?
ok, heart op. Numb your leg? Dentist!!!
Local? for a heart op? so they inject the leg, goes into the system, all out. its not a local!
I think your trying to say youve got a high resisance to anasthetic. local or central, but i dont get the question!
You dont have heart surgery with a local!

I had a deviated septum repaired and was out but awake at the same time. I could feel them drilling out my nostrils. It was probably the worst pain of my life.

When they woke me they said 'oh really, so that is why you were crying'. Idiots!

i did hear of such ppl esp at dentists you poor thing.
i had an epidural with my first son, which when administered hurt my brain real bad it did not work (unlike you i dont know what it is actually called) it all went very quick i gave birth but when they stitched me it was extreme agony i had to scream, she said how can it hurt you had epidural!
i had emergency section with 2nd son they gave me a spinal, i had a fear of being put to sleep, so i lied when they did a water test, they cut me 3 times i cried out, but told them to carry on they ripped me open but it was unbearable (comparable to torture) i could not handle it they were panicking unexpecting and rushing to ga me i lay there for moments all "open" how stupid was i? it took me a while to get over that one!
