Make sure you get a decent sized root ball. Also, make sure to loosen up the soil a little around the hole you place it in. It would be a good idea to wait until it is dormant as well (whenever trees lose their leaves in your area).

creeping phlox is very easy to transplant if you are separating it just cut it in half and place in soil and keep moist.
We used to take a clump of it and divide it in fours to make 4 plants out of it. As long as you keep it moist for a couple weeks while it takes root again you will be OK. This is a plant that is really hardy and hard to kill.

Yuo should be able to transpant it , just be careful of the roots as they are sensitive, make sure you mist it often, and dont get alrmed if you wake up it it goes limp-its normal for it to go through a shocking period of 1-3 days. Just keeping "misting".

dig up the whole plant, and cut threw the middle of it with a
shovel and plant. its really easy.
