Go 2 a recommended Solicitor.
Tell them U want 2 change UR name, U shouldn't have 2 offer them an explanation why.
Pay UR £50 or so.
Solicitor will do all the work 4 U.

Contact your local court house and ask about aquiring forms. In fact you may even be able to find them online. You will have to appear in court and sign docs that say you will still be responsible for the name you are giving up (like if there are unpaid bills or parking tickets in that name).

Go to a local courthouse and get the correct documents and paperwork for a name change (or find them online). It's different by state... (and sometimes depends on your age), but usually you have to pay a fee, get it notorized, etc.

You need to go or call your local Probate Court someone should be able to help you there. I don't think that you need a lawyer on this but someone in Probate should be able to help you with all your questions.
