Well u wouldn't need 1 unless u're jamaican. Either way at the passport office between twin gates plaza and the constant spring arcade (bring all neccessary documentation with u). If u live abroad u can go through ur local postal agency (altho it can take forever and a day). If u're a minor then a family member in Jamaica can do it for u.

Well a passport is good for all countries that allow US entrance. Therefore all you need to do is apply for a US passport which can be purchased at a local post office. or you can go to the link below and find out all neccessary information and where the closest place to obtain a passport is located for you.

Are you a Jamaican citizen living in Jamaica or a citizen from a different country??? If you are talking about thinking you need a specific passport for Jamaica you don't. You would only need a passport from your own country. If you are talking about a the possibility of needing a visa to visit Jamaica go to http://www.congenjamaica-ny.org/visas.htm and it will tell you if you need a visa to visit Jamaica from your home country.
