In the first instance the title is granted by the monarch in gratitude for some service rendered. From then on the title passes on from father to eldest son. If the eldest son dies without producing a son of his own the title goes to the next eldest son. If the holder of the title dies without having had any male children the title becomes extinct. Titles only pass on through the male line, daughters may not inherit. For a woman to be a countess it is by being the wife of a count.

Well, that title is for people of status (rich) and that I know of, it's only in europe, but I don't think it's used anymore...

by royal degree. A king or a Queen gifts those titles to their most faithful servant ( supporter).
