Good Question! Their brains don't fall out becuase open minded is simply a figure of speech. Being open minded means thinking "outside the box". Just becuase someone's mind is open doesn't mean their brains will fall out becuaes they may be standing perfectly still.

Because people would have to be open-skulled for their brains to fall out. In some people though, brains drip down from other orifices such as ears or the mouth. This might happen to you if you hear yourself speak.
