you always start with a theme or story that you feel like telling. however where it goes from there is an organic process, if you will. the key is not to censor yourself along the process. like most things in life enjoy the process with no specific goal or plan on the outcome. as a budhist once said it's the journey up the mountain, not the mountain top that's reached.
remember writing is perhaps the most difficult of all the art forms. re writing is a part of it, a huge part but don't concern yourself with that if you're just starting out.

It depends on the writer and story length. A story can take you anywhere and in a long story your theme may change over time.
You may to set goals for yourself pertaining to both theme and character development, once writing see how comfortable you are working toward those goals and adjust accordingly.

Theme should always be chosen first, because when you dont have one, you write about many things and digress easily. If you find out the theme by writing, you can start with one thing and end with another, then the essays or anything you write should be edited to cut out the things that have no connection to teh theme. So i believe thats its better to start writing when you have one, though it hardly works with me..

I think you should choose a basic idea and then let all the smaller themes grow on there own.
