Yes, there are several documented cases about people dying in heart failures in an extremely frightening situations; even if they didn't have a diagnosed heart conditions and were perfectly healthy. Naturally it's kind of hard to tell if there was some sort of hidden problem in persons heart _after_ it has failed, but this would require quite a lot of speculation.
Besides the shocking type of fear, causing heart failures, there's also the possibility to just die if you believe it hard enough. Just die out of fear of something. This phenomenon is sometimes called "Voodoo death" or "Hex death". I'll list a couple of articles in the sources, but basically it seems to be a kind of strong placebo effect, sometimes called as "nocebo" (like an "evil twin of placebo").
Makes you think about all the fears modern media is inducing. You can't eat this, you can't eat that, you get shot and mugged if you go outside, planes crash with or without terrorists, and if you manage to stay alive by fortifying your house, you still may get killed when a hurricane comes and rips your fortress apart.

I don't have a webpage for you, but while I was in Florida, there was a story on the news about a 12 year old boy who was literally scared to death by a black bear. Poor little thing!! It was later discovered that he did have a mild heart problem, but he probably would have been fine if not for the bear.

I don't know about web pages, but Cat; it's pretty well documented through out history that it's easy to scare some folk to death. The libraries must be on line so I would think a link to any historical section would get you there. Ancient Roman accounts are prolific in this sort of thing. Jeez, my ex wife scared me almost to death two weeks ago! She wasn't trying either, thank goodness! Her and the coven that She runs with could learn you a think or two, but you might not survive them!

Some people say that you die before hitting the ground if you jump off a tall building or high bridge?
I have no idea how anyone would prove that though, so... Lotsa help I am...

I don't remember ever hearing about "Scared" as being a category or code on autopsy forms.
How would you determine the cause of death?
