The best thing to do is to call your insurance company and see what your plan covers and doesn't cover, while you are talking to them they can give you names of providers that will do the procedure. It has been my experience (working for a surgeon) there is no such thing as a cheap surgery. You get a bill from the surgeon, bill from the hospital, bill from anesthesia, bill from pathology and who ever else sticks their hands in some way into the situation.

You really get what you pay for in this area. The doctor that invented the procedure of tubal reversal is close to the MI border, but I don't remember his name. The average cost is $6000.00 on the average and that includes everything. They have payment plans that you can "lay away" your tubal surgery by making monthly payments. It's worth it to look for and pay for some one most qualified, rather than the cheapest you can get. Some doctors guarantee their work, to the point of redoing the surgery for free if needed.

Most insurance companies won't cover reversal of a tubal.
When I trained in Conecticut, state welfare definitely wouldn't.

To be quite honest. Nothing is cheap in health care.
Any Dr. or clinic offering "cheap" surgical proceedures,
I would be quite leary of.
