Adnexal describes the areas on either side of the uterus, like your right lower quadrant and your left lower quadrant. Lesion is a word to describe a cyst or tumor, it ecompasses all pathology not just cancer. Most adnexal lesions are ovarian in nature.

Adnexa are the ovaries, fallopian tubes and ligaments to the uterus. If they are using the term "lesions", I think they may be talking about small foci that show up on sonogram and could represent endometriosis or (less likely) tumor.

If it is in the setting of an infertility workup, probably the endometriosis.

They are lesions that, for some reason, are adnexal.
