In what sense do you mean "authentic?"
Here are some possible interpretations of your question:

"Did Leonardo really capture the essence of his sitter? An authentic picture must accurately depict its subject matter."

"Did Leonardo even really have a sitter for this painting? An authentic picture must represent something real, not imaginary."

"Did Leonardo really paint the Mona Lisa between 1503 and 1507? If not then the painting is inauthentic."

"Was there really a painter named 'Leonardo'? Or was the Mona Lisa actually painted by someone else who we today call 'Leonardo'?" The painter must have referred to himself as 'Leonardo' at all times in order for the painting to be authentic."

We can be certain that the painting exists as an cultural artifact. The notion of authenticity can be applied to such a thing in many ways, none of which are completely knowable.
