There are two ways to do it -- city limit to city limit, or city center to city center. Some expense accounts allow one, others allow the other.
Race organizers usually go city limit to city limit, if it's a bike race or a distance walk.
A GPS is the easiest way to get a precise measurement, whichever point you decide to measure from.

Find out the latitudes of both the cities with the help of your atlas. Know the difference of both the latitudes. The distance in between two latitudes is 69 miles. Now you can easily find out the distance between both the cities by multiplying the difference of latitudes by 69. For example, city 'A' is on 38 degrees north and the city 'B' is on 42 degrees north. The difference between these two is 4 degrees. So the distance between these two cities is 4X69=276 miles. It's clear, I hope.

By rail it is the distance between the railway stations to railway stations.
By air it is the aerial route distance through which the air traffic is routed from one city to another city
