If you say "the wrong thing," you can always say you're sorry. You can learn from your mistake. If you say nothing, you can only guess what the reaction might have been.So, keeping your lip shut is worse.
Having said that, let me add that silence can be golden. Think before you speak.

this actually calls for two different and unique situation: "saying something & wishing you had not" often occurs in a fit of anger, it is often the time when a person is so overwhelmed with emotion and say things without thinking how their words will affect the person involved, while the other "saying nothing and wishing you had" happens when a person is experiencing regret and often it comes after a loss.. though both hurts, i'll say the last one hurt more because in the first instance you still have the chance to apologize for what have been said and though it might not change things much, the fact that you can still say sorry is a burden off your shoulders... while in the second situation, all that was left for you is regret and wishes that you will be given another chance (impossible if the person is dead)..

When we talk we have to evaluate the consequences of what we say before we say it. We must evaluate what we intend to convey and the results or consequences. Sometimes it hurts like crazy to break up with some one but it sets you and that other free to pursue something or someone better. What is the context of the question? abuse? Report it so it does not have to be endured by the abused another second. It would certainly be agony to know your lack of conscience caused extreme pain but in the interest of humanity if it is something else be kind so the result can be a win win situation for the good of all......Life is painful find a way to bring TRUE love and kindness to others. Do you think the Doctor hurts less by telling the truth than he would by ommitting?

Saying nothing. Tell everyone you love that you do love them before they are gone forever. I have regretted not telling people how important they were to my life and now they are gone. Now I make sure to even write letters to people from my past that I'm glad came into my life and why. I have even rekindled old friendships that way. Never hurts to say something nice. Just hold your tongue about the bad stuff - if you can.

I have done both of these things numerous times in my life and i have found that it is best to say nothing and wish that you had, because you can't take back what has already been said

whishing you had. At least you know whta happened after you said it and you can go back and fix it, but when you dont and that oppurtunity passes by you can never get that back cause sometimes you only get one chance.
