I would like to add an online application form to my website for people to fill out. Where can I get one?

I am opening a child care center and I would like to add an employment section for people to fill out. How do I get an online application and have it submited to my email???
Any advice is appreciated.

Yahoo has a forms section that you can make and fill out and they will make the code. You just copy and paste. But if that doesn't do it for you I could make one for you--for free. I do website coding as a hobby.
