She must get a 92.7 or better to get an A for the class.

86 * 0.4 = 34.4

90 - 34.4 = 55.6

55.6 / 60 = 92.6666...

40% (or 4/10) of 86% is 34.4% of your grade. That leaves another 55.6% to get you a 90%. If you divide 55.6 by .6 you get 92.667%.

Therefore you need a 92.667% on the exam to get an overall 90% in the class.

.4x + .6y = .9

Since we have "x", let's plug it in.

.4x = .4 * .86

.4x = .344

So,
.344 + .6y = .9

Simplify:
.6y = .9 - .344

.6y = .556

Divide both sides by .6

.6y / .6 = .556 / .6

y = .556 / .6

y = .9267

So, she needs to get a 92.67% to get an overall 90%.
