They actual pierce the lining of the hip hole! I was told to make anal sex more enjoyable! And yes wtf!Gross!

it either means there cool or a piercing on the hip or a "surface" piercing most places on the body can have a hole punched through it .... mine back of neck and webbing of thumb and fore finger.... now there hip piercings.. he he
