First, make a list of the writing habits you have. Then make an outline using them and plan out what you're going to say in each paragraph. Try writing a pre-draft too that you can give to a classmate or the professor to look over and make comments/corrections. Watch your spelling and grammar also.

This assignment is a questionairre--the prof is giving it to help steer the teaching to the class. Be honest and organize your thoughts. Think about the last few essays you had to write and how you did it. Throw in an example if you like, but don't get too specific or make a list of what you have written.
I doubt anyone who does the assignment will get a bad grade unless the prof thinks they didn't care. Check yourself for grammar mistakes, etc. You haven't been taught anything yet--so don't stress over it. The prof wants to know how much or how little you know.

first: brainstorm. figure out 3-4 things that Show and Prove what you are trying to convey to the reader. your topic is Writing Habits. how do you come up w/ the idea? how do u come up w/ your supporting paragraphs?
remember, an outline for ANY essay can be pared down to this:
Intro (including thesis statement)
Body (3-4 paragraphs specifically stated in the thesis)
Conclusion
generalize what u are trying to do in ur essay. that will be ur introduction. in ur thesis statement, u want to specifically state "my writing habits are very effective because they are A, B, and C".
then A, B, and C will be your body, and each paragraph will have specific examples of WHY they are effective.
good luck!

Think about how you get your ideas. Make notes..just write down key words like an outline. The first paragraph should be a summary of what your paper is about. Then each paragraph be a point from your first paragraph, the so on....second third...etc. the last paragraph should conclude what you've talked about bringing it all together.

Just write it. Then get a friend to critique and edit it. Revise. Revise. Revise.
2 pages isn't so bad. I'm sure you can do it.
