Classic transmissions or gear box problem.
Being that it's a conversion van, it's most likely your gear box.
Either way, it's a costly problem.

Without knowing the year, make, and model the suggestions are limited. However you seem to be having a linkage or shift cable issue.

you can break down the problem by disconnecting the linkage or cable that connect to the transmission.now shift the handle and see if i move easy.if it move easy,you know you have a trans. problem

Either your tranny needs to be serviced or your shift mechanism is getting worn out.
