Medicare is 1.45% of wages
Federal withholding would depend on a couple of factors such as marital status and dependents. Pay frequency also has some effect. You can access the withholding tables at http://www.irs.gov Publication 15
State withholding also varies by state and dependents you can probably access state tables by searching the department of revenue for the state in which you reside.

I always use 15% as may estimate to cover all my tax obligations. It has worked pretty well for me. So say, I was to gross 1000 per payperiod. I would multiply by .85 ( that's 1 minus .15). I would build my budget form that point and adjust once I got a full paycheck as necessary. Keep in mind, your filing status and number of exemptions will have an effect on your amount of tax. Sinlge takes more, married takes slightly less, etc.

Depends on your base salary and how many withholdings you elect to take on your W4, but ballpark deductions are between 20-40% (inc. SS contributions)

Bank on about 28-33%. Here is an example: $1500.00 paycheck really means about $1100.00 in your hands. This is when you claim ZERO exemptions.
