My vote for worst, Crete. Greek plumbing means no tissue can be put down the toilet resulting in vile baskets full of paper. The best were probably in Iceland.

Worst - Thailand, same for the baskets of paper scrap but they are squatters, no seat to sit on! (Not all places just older ones eewww!
Best is Japan, they have electric "washlets" that have any or all of the following features: heated seats, bidets to wash your bum with varying pressure adjust, electric dryers for your tush, perfume spritz, plastic wrap that autmatically puts new plastic on the seat as you sit down (no sitting in strange puddles), and calming babbling brook sounds to relax you while taking care of business. I am not kidding I have seen & tried all to differing approval, but they are all real!

Though I am by no means particularly well-traveled, I've been to a pretty decent variety of places. Toilet facilites will vary widely, no matter where you go. It'll always be a mixed bag.
