your surgery will send your notes to the practice you end up going to (i work in a surgery) you have got the right to a print out ( a short summary) of your med notes with up to date imms, vaccinations on etc but u will not be allowed your mre. which is your little folder of all your medical history and stuff.

Yes, if you've ever been registered with a practice they legally have to supply you with your medical notes irrespective of where you are. Do it in writing.

Yes; you have to put it in writing to you local hospital/authority as the case maybe.

be prepared for it to take a long time.

Explain why, they may be a bit quicker if they know that you're not taking them to court. :-)
