If you insert an image into a HTML email the recipient will only see the image if a) their email client allows for the viewing of HTML emails and/or b) if they are using outlook they will be asked if they want to view it.
Many spammers use images in emails as a way of tracking whether an email address exists. They put the image on their server and link to it using HTML in the email - this allows them to gather statistics (and email addresses) from their server. Therefore as standard Outlook asks the user if they trust the person who sent the email before they allow for the image to be shown.
