First of all, appraise yourself first. Find out what you've done for the company in the last 6 months to 2 years that has helped in the company's business.
Analyze your own strength and weakness. If you have done well as mentioned above and have more strengths than weaknesses and are prepared to do something to improve in order to minimize your weaknesses, you're in a good position to ask for a raise.
Find a suitable time when your boss is in a more relaxed mood. Always approach your boss for such discussion on Fridays!
Tell him frankly your concern about your position and future in this company before giving him the reasons why you deserve a raise.
