No, they don't need air. The sewage system should be vented at your house. Most manhole covers are loose and just sit on top of the manhole because they are out in the open, but some, typically ones in streets, are bolted down.

Nope there should be air vent on the line somewhere and it must be elevated use ally about 3 meters high so you wont smell the gasses that been released. If the manhole covers are the small lightweight plastic type make sure its screwed down properly and if ut the heavy cast iron type its ok as long as the real air vent are in place or the gasses will escape there and it will surely stink
