There are great recreation programs out there for people with disabilities. Two of my favorites are BORP and ETC.

I work with mildly handicapped persons and we take them horse riding ( the is an association here in australia the has riding for the disabled) , swimming , bowling, making crafty things - there are a few suggestions - hope this helps

I teach TAI CHI CHUAN. A good instructor could make a fitness plan for people with a verity of problems, even wheelchair TAI CHI.
