Loose, definitely, and properly made in a warm pot of the correct shape and an appropriate size. I use single-estate broken orange pekoe tips when I can find them, but Twinings English Breakfast is good for when I can't.
As for what I serve it in, I'd like to say a fine china cup and saucer, but then I'd be up and down like a yo-yo constantly refilling it, so I use an elegant mug.
Frankly, and I don't care what anyone says, if I use tea bags then I can taste the paper.

I think tea is much better loose but I always spill it all over the place so I use bags. I have a special tea pot given to me by a friend and I always use that and one of my special cups cause I don't drink it that often. I think Chamameil(sp?)is my fav.

Bagged - Asda's own - and I serve mine to myself in a tall black and white mug shaped a bit like a vase, i.e. wider at the top than at the bottom.
For tea 'types' I like Assam whilst my husband prefers Darjeeling.
